Your Incident Report records the circumstances around an interruption to a contracted paid performance.

Sh*t happens. And unfortunately, it often happens at a gig. That’s why we recommend following your own Risk Management Plan and adhering to your Safe Work Method Statement (SWMS) at all times.

If you cannot complete a contracted paid performance, refer to those plans to identify what happened, where the response fell short and what measures could help prevent a repeat.

Explain why the incident could not be successfully managed within your own Risk Management Plan or SWMS, and why you could not continue performing.

Upload your photos or videos, incident log and SWMS to Google Drive or Dropbox, then provide accessible links in the form. Pushworth will forward your Incident Report to the client.
